All the Important Forms to be filled by such Staff upon joining SUA. Kindly fill out the following forms, which are provided to you upon your arrival
- Personal Record Form
- Medical Forms (for a Health Check from a designated or recognized Medical Facility)
- EB1 Form regarding Public Service employment
- Access to mailing system and other online services (such as ESS, Biometry, EDMS, MUSE, NEST, Staff mailing, SUARIS, PSSSF, etc)
- Form for joining SUA Health Supplementary Health Scheme(Voluntary), NHIF( Health Schemes for You, Spouse, Parents and up to four Children/Dependents
- Form for Staff identity card
- Form for Public Service Social Security Fund
- Orientation form Manual: This form has to be filled by the new employee and the supervisor at the working station upon completion of orientation
